personal federal income tax returns (with all schedules) for the past two (2) years.
7. If you are self-employed or own your own business, business
federal income tax returns (with all schedules) for the past (2) years.
8. Documentation of any other income (child support, alimony, trusts, etc.)
to be considered as part of your monthly income.
9. Name(s), addresses, account numbers, and approximate balances for all checking and savings accounts.
10. Statements for
bank accounts, credit unions, certificates of deposit, and a brokerage accounts for the past three (3) months.
11. List of savings bonds (serial numbers) and
approximate values.

List of stock securities and approximate values.
13. List of life insurance policies, amounts of coverage, cash value, and monthly cost.
14. Year, model, and approximate value of automobiles
owned.
15. Estimated
value of furniture and other personal effects owned.
16. Documentation of any other assets you wish to have considered as part of your net worth.
17. Names, addresses, account numbers, and approximate
balances for any mortgages or other loans (such as an equity line of credit or second mortgage) secured by real property.
If the property is to be sold or rented, a copy of the sales contract or lease.
18. Names, addresses, account numbers and approximate balances
for any installment or revolving credit accounts (the cities including credit cards, personal lines of credit, student loans,
and automobile or other vehicle loans).
19. Documentation of any other liabilities (alimony or child support, old, payments against unresolved
judgments, etc.).

Amounts currently paid as housing expenses, including principal and interest payments, home and/or mortgage insurance premiums,
property taxes, and home owner’s association dues.
21. If you are divorced or separated, a copy of decrees or agreements.
22. Letter of explanation with supporting documentation
in any case of credit delinquency.

If you’re purchasing a home, a copy of the real estate purchase contract and documentation of any earnest money deposit.
24. If you are refinancing
your current home, a copy of the deed and the house location survey of the property.
25. You also should know the year in which the home was built,
the year in which you originally purchased it, and its cost when purchased.
26. To use a gift as a source of the funds required for closing, a copy of
the gift Letter and documentation as to the availability of those funds.
27. Veterans applying for a VA mortgage loan will need an original VA certificate
of eligibility. Active duty personnel also need a statement of service Letter. All others will need a copy of DD form 214.
28. A check for
the application fee, covering the cost of the appraisal and the credit report.
